Procedure to follow for filling the New Connection Online Application Form
1.Register your mobile number using OTP
2.Fill in Application and connection details and click on "Save".
3.Upload Self Attested documents in pdf format of file size not exceeding 2MB each.
4.After Uploading documents, note Application Number and Transaction ID for making payment of Rs 150/-(Form fee Rs.50/- and processing fee Rs.100/-) (Using Application Number and Transacation ID).Kindly note that this is a non-refundable amount, so avoid doing multiple payments in case of payment failure.
5.Make payment using Application Number and Transaction ID.
6.After payment Click on FINAL SUBMIT. Notification Number will be sent on your registered mobile for your future reference.
7.You will receive Email and SMS on your registered mobile Number - 1 ) Acknowledgement of application.
8. Email and SMS will be sent on your registered mobile Number for further reference.

(iii) Driving license
(iv) Photo identity card issued by Government agency
(v) PAN card
(vi) Certificate from village Pradhan/Patwari/Lekhpal/village levei worker/village chowkidar/Primary school teacher/in-charge of primary health center etc.
(vii) Aadhar Card

If Applicant is an organization(Any one of the following)
Signature of competent authority (e.g.Branch Manager, Principal, Executive Engineer etc.) along with relevant resolution /authority letter of the institution concerned.

Proof of Ownership*

1.Registered sale Deed/Lease Deed of the Premises/Form I & XIV
2.Municipal / Panchayat Tax Receipt indicating house number and the name of the owner of the premises
3.Letter of allotment
4.Copy of the House Registration Certificate issued by the Panchayat/ownership certificate issued by Revenue Authorities
5.Succession Deed/ Inventory Proceedings/Registered will
6.GST Registration reflecting name and address of the premises
7.In case where an occupant is unable to produce any of the above documents then an order under the Goa Health Act, issued by the concerned Health Officer may be submitted.

Proof of Current Address

(i) Electoral identity card
(ii) Passport
(iii) Driving license
(iv) Ration card
(v) Photo identity card issued by any Government agency
(vi) Statement of running Bank Account
(vii ) Water/Telephone/Electricity/Gas connection Bill
(viii) Income Tax assessment order/Aadhar Card

Photo of installation where supply will be released
Document of Any outstanding dues in any case
Test Certificate and Wiring Diagram from Licensed Electrical Contractor to be compulsorily submitted before release of connection  
Any other document as applicable  
Note:
Any document of proof of identity and address such as Electoral Identity Card , Passport, Driving License, Photo identity card issued by Government agency, PAN card, Aadhar Card may be uploaded in case any of the document mentioned at Sr. Nos 1 to 7 does not reflect identity and address details.
